- Required Health Information Portal (ALL students must complete this by the start of their first fall semester to be eligible to register)
- Independent Study Form (PDF) – due by the first day of the semester you are completing the independent study. This is filled out for either campus.
If you are completing one at UNC Greensboro, you will also need to fill out the Permission to Register for Independent Study form (web-based form). You must coordinate with the department before you can fill out this form as you will need the instructor-specific CRN and section information. - Joint Programs Cross-Registration Information
To register for a non-Joint M.S.W. elective course only – speak with your advisor and the program director about this option!

Field INSTRUCTION Forms
The field instruction program is an integral part of the Joint M.S.W. curriculum. The field program is administered by the Co-Directors of Field Instruction at NCA&T. Field-related policies, procedures, and practices are explained in the Joint M.S.W. Field Instruction Manual.
Two-year students are in the field 4 semesters for a total of 1112 hours. Advanced Standing students are in the field 2 semesters for a total of 672 hours.
Students are placed in different settings for their generalist and specialist placements.

Template Options
A Plan of Study is your intended outline of the courses you will take to complete your degree. They are required. Plans of Study are due before the completion of 18 semester hours. Advanced Standing and new Two Year students will submit these their first fall semester.
Revised copies (if needed) are submitted in January of the semester you graduate.
Steps
- Download the copy that best suits your status. Type directly into the document to complete it.
- Fill out the top section with your information; do not forget to click the drop-down menu and select your expected graduation semester.
- Do not change the School/College, Major/Major Code, Major boxes, or Credit Hours boxes.
- After you fill out your form, follow the Program Director’s instructions for signing and turning in (you only turn in a Plan of Study for your home campus).
- Keep a copy for your records so that if you do need to revise, you won’t have to start completely over.
NCA&T PLAN OF STUDY (for NCA&T-assigned students only)
Fill in your advisor’s name and email. Do not put anything in the “Graduate Courses Completed at Other Institutions” boxes on the first page – UNLESS you actually have transfer credit.
Add expected year of completion on the table that documents the coursework (the semester is already there, just click to the right of the semester and type in the year). You can change the electives (default is SWK 685 Social Work Special Topics) but all the other courses are required and will require approval to change.

Add expected year of completion on the table that documents the coursework. You can change the electives (default is SWK 654 Special Topics in Social Work) but all the other courses are required and will require approval to change.
Template Options – Word or PDF
These templates may not cover every situation but should work for most cases. If you have transfer credit or other unusual situations, you may need to make additional edits to accurately document your coursework.
Use the Adobe PDF to capture electronic signatures using an Adobe Digital ID. Do not lock the file after signing or you will not be able to have your advisor and/or the Program Director sign.
Use the Word docx file if you plan to print to obtain physical signatures or need to make significant edits to the coursework, use one of these.
